What Is Chronic Care Management?
CCM is a Medicare-covered program that gives patients with two or more chronic conditions access to dedicated care coordination support for at least 20 minutes each month. It is delivered between your regular office visits, which means help is available when you actually need it, not just when you happen to have an appointment.
Through CCM, a dedicated care manager works with you and your care team to:
- Build and maintain a comprehensive care plan tailored to your conditions
- Coordinate referrals and follow up with specialists on your behalf
- Help you understand your treatment plan and stay on track
- Address barriers to care, such as transportation, insurance, or prescription access
- Track your health goals and adjust the plan as your needs change
Who Qualifies for CCM?
If you are a Medicare patient with two or more chronic conditions expected to last at least 12 months, you very likely qualify. Common qualifying conditions include:
- Type 2 diabetes
- Hypertension (high blood pressure)
- Heart disease or heart failure
- COPD or asthma
- Depression or anxiety
- Arthritis
- Chronic kidney disease
Many patients are surprised to learn they are eligible and that Medicare covers most or all of the cost.
What Does CCM Look Like Day to Day?
When you enroll, you are paired with a care manager who gets to know you and your health history. They reach out proactively, check in on how you are doing between appointments, and serve as your advocate when navigating the healthcare system.
